循環器専門医研修単位3単位（区分：その他）取得について
本フォーラムに参加された方におかれましては、日本循環器学会循環器専門医研修単位を3単位（区分：その他）ご取得いただけますが、参加登録のみでは単位付与ができません。
当日会場にてご自身でお手続きいただく必要がございます。
単位付与をご希望される参加者の先生方におかれましては、専門医カードをご持参のうえ、日本循環器学会の単位登録受付にお越しいただき、単位登録の手続きを行っていただきますようお願い申し上げます。

12月13日（土）9:00-17:00 12月14日（日）9:00-16:00
- 指定された時間・場所以外での単位付与は認められません。
後日、ネームカード等での単位登録はできませんので、必ず時間内にお手続きをお済ませください。
Registration information
After completing your registration, visit your personal page "Payvent Hub" at the following URL: https://app.payvent.net/hub
At the venue, please come to the registration desk to pick up your namebages by presenting your QR code.
Both the QR code and the receipt are available on your personal page.
Once you have checked in at the venue registration desk, you can issue your certificate of participation.
For onsite registration, both registration and payment must be completed via the websit using a credit card. Please note that bank transfer or cash payment are not accepted.

On-site registration is also open during Congress period.

Online Registration Procedure
Please click on the "Online Registration" link at the bottom of this page and follow the instructions.
After successful registration, a confirmation email will be automatically sent to the registered email address.
Note: Registration via telephone, fax, or email is not accepted.
Method of Payment
Both credit card and bank transfer are accepted for online registration.
A unique bank account number will be automatically generated and assigned to each application.
Cancellation
No refunds will be made after registration is completed.
